You may need someone to help you with this activity. You will need to download this PowerPoint presentation of the Student Health Survey

You will need to answer 4 questions for each healthcare topic. As you answer the survey questions, add up your score using the score chart at the bottom of each PowerPoint slide.

Once you have your total, colour in the correct number of bars on the pie chart of your Health Survey Results sheet that you printed in Lesson 3a. If you don’t have a copy, here is the Health Survey Results sheet.

In the pie graph, each bar represents 2 points. So if you got 6 points for Attitude when you did the survey, starting from the centre of the pie, you would colour in 3 bars in the Attitude section.
